NATIONAL GIRLS AND WOMEN IN SPORTS DAY PREGAME CLINIC

Date: January 25th prior to Lady Raider Basketball vs. Oklahoma State at 2:00pm

Venue: Womble Basketball Practice Facility

Check-In Time: 11:30am

Clinic Time: 12:00pm-1:30pm

Participants will receive ONE FREE gameday ticket to the Lady Raider vs. Oklahoma State game at the United Supermarkets Arena right after the clinic on January 25th.

Walk ups will be required to pay full price for game ticket(s).

**Required to participate in the clinic**

I am the parent/guardian of the participant(s) in the Texas Tech Pregame Clinic. It is my understanding that participants in the activities that makeup the Texas Tech Pregame Clinic are not without some inherent risk of injury.

As such, inconsideration of the participation in the Texas Tech Pregame Clinic, I hereby release, waive, discharge and covenant not to sue the clinic, the Athletic Department, Texas Tech University, The Texas Tech University System, the State of Texas, their officers, servants, agents or employees from any and all liability, claims, demands or injury, including death, that may be sustained.

Whether caused by negligence of the releases, or otherwise while participation in activity, or when in, on, or upon the premises where the activity is being conducted.

I give my permission for any emergency medical care or treatment by a physician, surgeon, hospital, or medical care facility that may be required and accept responsibility for the cost. If at any time it is necessary for the aforementioned participant(s) to receive outside or professional medication, I hereby give my consent to the Clinic to secure transportation as it is deemed necessary.

I give my permission for any photographs and/or videos of him/her being taken on behalf of Texas Tech Athletics. I also grant the right to edit, use and reuse said images/videos for non-profit, non-commercial purposes, including in online, social media and all other forms of media. I give this authorization without expectation of compensation
